Below are the unique links to each participating library’s Book to Action 2021 Community Survey. This survey is required for LSTA funded initiatives, including Book to Action. Please encourage completion of the survey by community members.

Survey question text and order of questions may not be modified in any way. Surveys are available in English & Spanish.
Respondents should select English or Spanish from the drop down menu at the top upper right. See image at right.

We recommend sharing the survey link on your Book to Action URL, via email, via virtual program chat, at the end of each event, and at the end of your series. Upon completion of the survey, community members will be redirected to the Redirect / Book to Action URL listed. See the Community Survey Link column to locate your library’s unique survey link.

Community surveys should be closed a maximum of two weeks after your final program or by the end of July, whichever comes first. When your programs are complete, please notify Jasmin, Cc Holly and Julianna. You’ll receive a summary of your surveys from Jasmin. These survey reports will also be shared with California Center for the Book and utilized in the end of year report.
